2010 Open Enrollment Information for City of Modesto employees and retirees is now available.
Open enrollment is the one time each year that you are able to evaluate your healthcare needs and,
if necessary, make changes.
Important Dates
- The Open Enrollment Period will begin Thursday, October 1, 2009 through Friday, October 30, 2009
- All changes must be returned to the Human Resources Department by 5:00pm on Friday, October 30, 2009
- Elections made during open enrollment will become effective January 1, 2010.
- Open Enrollment Benefits Fair will be held at Tenth Street Place, Room# 2001 on Tuesday, October 6, 2009 from
8:30am - 5:00pm.

This is new. This open enrollment we are scheduling one-on-one appointments with employees who have follow-up
questions to the open enrollment meetings or need assistance completing paperwork. These meetings are limited to 15
minutes, so please come prepared with your questions and dependent information. Appointments will be available from
October 12th – October 20th. Please list your name, contact
phone number, dates and times that work best for you, and where you work (i.e. Corporation Yard, Police Dept,
Tenth Street Place, etc)
Health Insurance Information
Health Savings Account (HSA)
A Health Savings Account (HSA) is only available to employees enrolled in a HDHP medical plan (Blue Shield HDHP or
Kaiser HDHP). A HSA is a tax-favored account that allows you to set aside funds to save and pay for qualified medical
expenses incurred by you, your spouse, and any of your qualified dependents. The HSA is an individually-owned account
that earns interest and the funds are carried over, without limit, from year to year.
Flexible Spending Accounts
A Flexible Spending Account (FSA) is a tax-favored program offered by the City that allows employees to pay
for eligible out-of-pocket health care and dependent car expenses with pre-tax dollars. The City offers three
FSA accounts: Unreimbursed Medical (URM), Limited Expense Unreimbursed Medical (LEX URM), and Dependent Care
Assistance Program (DCAP).

Dental/Vision Plans
Chiropractic/Mental Health Plans
American Specialty Health (Chiropractic)
The chiropractic coverage through American Specialty Health is for those employees/retirees who are
enrolled in Blue Shield HMO, Blue Shield Traditional PPO, Kaiser HMO, or any of the Retiree Medicare
Advantage plans. For those employees/retirees enrolled in any other medical plan, the chiropractic
benefit would be through the medical plan. Employees who opt out of medical coverage do not have
chiropractic coverage through City of Modesto.
PacifiCare Behavioral Health
The mental health coverage through PacifiCare Behavioral Health is for those employees/retirees who are
enrolled in Blue Shield HMO or Blue Shield Traditional PPO. For those employees/retirees enrolled in any
other medical plan, the mental health benefit would be through the medical plan. Employees who opt out of
medical coverage do not have mental health coverage through City of Modesto.
