New Utility Services - Phone Sign-up

This form is to be used as part of the Phone Sign-up process.  Customers who signed up for service by phone should use this form to submit required documentation to complete the process.  

Once your documents have been successfully submitted you will receive an email with a confirmation number. To complete the sign up process and activate your utility account, you must contact the Customer Services Division.  For same day activation, you must speak with a representative before 3:00 PM.

Any past due balances along with a security deposit must be paid in full prior to establishing new services.
